If your email reset request has failed, you can use this form to request the EMIS Unit to add a secondary/recovery email to your account.

Fill in all the required details to generate your request letter
Next Steps After Download:
- Print the downloaded PDF and sign it
- Make a photocopy of your ID card
- Attach the ID card photocopy to the letter
- Submit both documents to the EMIS Unit at Thapathali Campus in person
